An analysis on pathogens and their drug resistance in periprosthetic joint infection

Abstract: [Objective] To analyze the characteristics of pathogens and their antimicrobial resistance in periprosthetic joint infection secondary to total hip arthroplasty (THA) and total knee arthroplasty (TKA) . [Methods] A retrospective study was conducted on the patients who suffered periprosthetic joint infection with positive results of bacterial culture subsequent to THA and TKA from 2016 to 2018. The characteristics of pathogens, antimicrobial sensitivity and minimum inhibitory concentration of various antibacterial agents to the pathogenic bacteria were analyzed and compared. [Result] A total of 295 strains of bacteria were isolated from the samples of periprosthetic joint infection, of which Gram-positive cocci accounted for 80.00% (236/295) ,Staphylococcus aureus accounted for 26.78% (79/295) . The detection rate of methicillin- resistant Staphylococcus aureus (MRSA) was of 27.85% (22/79) , while that of coagulase-negative Staphylococci was 45.08% (133/295) , methicillin-resistant coagulase negative Staphylococci (MRCoNS) was of 79.70% (106/133) . The sensitivity of MRSA was 86.36% (19/22) to gentamicin and 90.91% (20/22) to cotrimoxazole respectively. However, the sensitivity of MRCoNS was 84.91% (90/106) to rifampicin and 86.79% (92/106) to tetracycline respectively. All Staphylococci isolated were susceptible to linezolid and vancomycin.[Conclusion] The main pathogen of periprosthetic joint infection is staphylococcus and coagulase- negative staphylococci accounting for the highest proportion. The detection rate of MRCoNS is much higher than MRSA, which needs clinical attention.In addition to being sensitive to vancomycin and linezolid, Staphylococcus is also sensitive to rifampicin, levofloxacin, and moxifloxacin, which can also be used as an alternative in anti-infective treatment.
