Percutaneous pedicle fixation combined with debridement and implantation of sensitive antibiotics loaded calcium sulfate through quadrant channel for spine infection

Abstract: [Objective] To evaluate the clinical outcomes of percutaneous pedicle screw fixation combined with debridement and intervertebral implantation of sensitive antibiotics loaded calcium sulfate through the quadrant channel approach.[Methods] A retrospective study was conducted on 18 patients who underwent percutaneous pedicle fixation combined with debridement and intervertebral implantation of sensitive antibiotics loaded calcium sulfate through quadrant channel for spine infection from July 2015 to December 2018 in our hospital. The documents were analyzed, involving operation time, intraoperative bleeding, bacterial culture consequences, hospital stay, and variations in visual analogue scale (VAS) for pain, Oswestry disability index, erythrocyte sedimentation rate (ESR) , C-reaction protein (CRP) and Cobb??s angle measured on radiographs. [Results] All the 18 patients had surgical procedures performed successfully without serious intraoperative complications, and got primary incision healing. As results of bacterial culture by the pus and tissues, Staphylococcus aureus was found in 7 patients, Escherichia coli was proved in 1 case, tubercle bacillus was seen in 2 patients and negative culture result was in 8 patients. During follow-up period ranged from 8 to 26 months, no recurrence of spine infection was noted in anyone of them. At the latest follow up,statistically significant improvements in VAS and ODI scores, as well as ESR and CRP were noticed compared with those before operation (P<0.05) , although the Cobb??s angle measured on X-ray films remained unchanged (P>0.05) . [Conclusion] This percutaneous pedicle fixation combined with debridement and intervertebral implantation of sensitive antibiotics loaded calcium sulfate granules through quadrant channel is a sound management for spine infection, has benefits of maintaining the spinal stability while releasing effective local antibiotic to control infection.
