Double head threaded cannulated screw for Garden type Ⅲ-Ⅳ femoral neck fracture in nonelderly adult

    Abstract: [Objective] To investigate the clinical outcomes of internal fixation with the double-head threaded cannulated screws for unstable femoral neck fractures in nonelderly adults. [Methods] From January 2013 to July 2016, 32 patients aged under 65 years received internal fixation with double-head threaded cannulated screws (QWIX 7.5 mm) for Garden type Ⅲ-Ⅳ V femoral neck fracture, including Garden type III in 27 and Garden type IV in 5 patients, involved 19 right hips and 13 left hips. The operative time, fracture healing time, femoral neck length, neck shaft angle, femoral head morphology, internal fixation, and postoperative complications were recorded. [Results] All the patients underwent operation smoothly with operation time of (65.25±21.33) mins ranged 50 to 90 mins, blood loss of (65.51±15.36) ml ranged from 50 to 90 ml and hospital stay of 7.35±1.25 days ranged from 5 to 13 days. All 32 patients were followed up for 6 to 24 months with an average of 11.36±4.81 months. Of them one patient (3.12%) got fracture nonunion and 3 patients (9.37%) had femoral head necrosis. No screw withdrawal or femoral neck shortening happened in anyone of them. The horizontal shortening of the femoral neck [ (5.80±2.51) mm versus (5.22±2.83) mm, t=-1.723, P=0.389 ] and vertical shortening [(7.53±2.14) mm versus (7.35±2.95) mm, t=-1.502, P= 0.780] kept  unchanged compared the data at 1 year with those at 2 years after operation. The Harris score for the hip was recorded 66~92 points with an average of (85.92±3.65) at 2 years postoperatively, corresponding to the result grades, which was excellent in 19, good in 8, fair in 3 and poor in 2 patients with excellent and good rate of 84.37% (27/32) .[Conclusion] This double head cannulated screws is reliable internal fixator for unstable femoral neck fracture in nonelderly adult with advantages of avoiding screw withdrawal or shortening of femoral neck, decreasing incidence of complications, and improving fracture healing.
